I understand that required travel-document information must be provided by May 1, 2027. Failure to meet participation requirements will be handled under the WAVE Italy Terms & Participation Agreement.*

I understand that the participant must have travel insurance valid in Italy, including emergency medical treatment, evacuation and repatriation, with coverage for organized volleyball camp activities. Proof is due by May 1, 2027.*

I confirm my child is physically fit to participate in high-level volleyball training.*

As the participant’s parent or legal guardian, I authorize Via Varsity designated program representatives to seek necessary emergency medical evaluation and treatment for my child if I cannot promptly be reached, as described in the Terms.*

I understand international travel requires valid passport and travel documentation.*

I understand this is an international residential program and that athletes will stay in shared accommodations under staff supervision.*

I have read and agree to the Wave Italy 2027 Terms, Waivers & Participation Agreement,  including the payment and cancellation policies. I confirm that I am the participant’s parent or legal guardian, or an adult participant accepting for myself.*
